03Finding

Staffing shortages don’t create problems. They expose them.

When a laboratory runs short-staffed, the shortage reveals what was already fragile: information that was never documented, policies that couldn’t be located, handoffs that depended entirely on memory and goodwill.

The shortages didn’t break anything. They removed the buffer that kept weaknesses invisible.

04Finding

The costliest failure point is the shift handoff.

Every result depends on dozens of conversations and decisions, and the shift transition is where the most information is lost. Verbal handoffs drop detail, notes get misread, and the incoming shift discovers mid-shift that reality differs from what they were told.

Structured, consistent handoffs are not a process improvement. They are a patient safety issue.

05Finding

Inspection readiness is built daily — or assembled under deadline.

Inspection prep too often means weeks of chasing competency sign-offs and reconciling policy versions by hand. The labs that pass with the least disruption are simply better at operating in the months between inspections.

Compliance shouldn’t be a two-week sprint. It should be a byproduct of daily operations.

06Finding

The gap was never the LIS.

The LIS moves orders, tracks specimens, and reports results — exactly its job. It doesn’t know about the missed handoff, the outdated policy version, the expiring reagent, or the improvement project that stalled between meetings.

The laboratory doesn’t need another LIS. It needs the layer around it.

What Laboratories Did
